I'm not sure how it works for other LJ layouts, but for mine I go to the Customize Journal page and go to "sidebar" (in some layouts I think it's called "components", in others they don't allow the option). Then it's just a matter of putting the image code ( without the final space) in the text box.
Unless you meant the actual profile page? But there again, it's the same deal - putting the image code in the same box that you type in your profile.
